NOTICE TO ALL PARENTS

Your child's enrollment with the Jefferson Avenue Childhood Development Center is contingent upon the completion of the following requirements:

2. Complete registration card/blue card

3. Complete current physical within the last 6 months

4. Complete application for Free or Reduced Meals

5. Interview and tour of Center

6. Signed Parent Handbook

7. Complete "Child Care Request" form (if applicable)
